The new requirements to request the Single Disability Certificate

The Municipality of the City of Mendoza, public department in charge of issuing the Single Certificate of Disability (CUD)reported the new requirements established by the National Disability Agency (ANDIS) to request the document.

In this sense, ANDIS is the one that certifies the existence of disability, its nature and degree, as well as the possibilities of rehabilitation of the affected person.

In turn, taking into account the personality and background of the application, it will indicate what type of work or professional activity you can perform. This document is what will be called “Single Disability Certificate”, and It is issued so that these people can access all their rights.

To request a shiftit must be done in a manner in person in street Las Heras at 745of Monday to Friday, from 8:30 a.m. to 1:30 p.m.

New requirements

The new conditions established by the National Disability Agency are the following:

• Photocopy of ID.
• Photocopy of CUIL.
• Photocopy of complementary studies, such as: reports, clinical history, x-rays, audiometry and different studies depending on the diagnosis. (Up to 12 months old).
• Original medical certificate with diagnosis from the treating doctor. (diagnosis, treatment, medication, rehabilitation, necessary equipment).
• Complementary forms (depending on the diagnosis).
• Photocopy of birth certificate (if under 16 years of age).
• Photocopy of ID of mother or father (if under 16 years of age).
• In case of renewal, the expired or soon to expire certificate is also required.
• If the person is hospitalized or in a residence, it is requested that it be specified in the certificate and separately in a survival certificate, dated 1 or 2 days before the shift.
• If the address appears in the City, you must present said documentation in Las Heras and Peru, from 9:30 a.m. to 1:30 p.m., for the Disability Evaluation Board.
• In the case of theft or loss, you must file a report at the police station or civil registry.
